تحليل نقل الدم هو CROSS MATCH
اولا يتم عمل BLOOD GROUP لمعرفة فصيلة دم المريض ومن ثم يتم اختيار كيس دم من نفس فصيلة دم المريض و وعمل توافق الفصائل
وطريقه CROSS MATCH
ناخذ سيرم( مصل المريض ) قطرتين ونمزجها مع خلايا المتبرع (قطره واحده ) بعد ان يتم غسل الخلايا ب المحلول الملحي NORMAL SALINE ثلاث مرات ويتم وضعها في انبوب وهذه الطريقه بالتفصيل
Use fresh blood ( less than 48 hrs old)
IMMEDIATE SPIN PHASE OF MAJOR X-MATCH
Label a glass tube •
Prepare 3-5 % saline suspension of washed donor red cells to be tested •
Add 2 drops of patient serum •
Add I drop of donor red cells suspension•
Mix and centrifuge for 15-20 seconds at 3000 rpm•
Examine for hemolysis and gently resuspend the red •
cell button and examine for agglutination
Read , interpret and record the result •
Agglutination or hemolysis = incompatible
A smooth suspension of red cells after re-suspension of red cells button = compatible immediate spin cross match
ALBUMIN PHASE FOR MAJOR X- MATCHING
Add an equivalent volume of 6 % or 22 % of bovine albumin •
Incubate at 37o C for 30 min. never incubate for more than 60 min •
Centrifuge for 15 seconds at 3000 rpm •
Examine for heomlysis and agglutination , grade and record the results •
No agglutination = compatible
Agglutination or hemolysis = incompatible
INDIRECT ANTIGLOBULIN PHASE FOR MAJOR X- MATCHING
Wash the cells 3-4 times with normal saline and completely decant the final wash
add anti-human globulin to the dry cell button( 2 drops)
or according to manufacturer's direction
and mix well
Centrifuge for 15 seconds at 3000 rpm, observe the agglutination, grade and record the result, examine negative under the microscope.
No agglutination = compatible
Agglutination or hemolysis = incompatible
Note : to all negative tests add one drop of coomb's control cells ,centrifuge for 15 seconds at 3000 rpm
Resuspend the cells and examine
If agglutination occurs = valid test
No agglutination = invalid test to (be repeated )
