Blood smear

We use blood smear for evaluation of morphology of
:

white blood cells (differential white blood cell count)
red cells
platelets

Indications for blood smear analysis

WBC < 3500/ul – all cases
WBC < 4000/ul – patological WBC differential count
WBC > 30 000/ul – all cases
Hepatomegaly
Splenomegaly
Lymphocyte > 4000/ul
Neutrophils <2000/ul
PLT < 100 000/ul (140 000/ul

Blood smear – specimen

fresh drop of peripheral blood (without anticoagulant)
smears blood must be made immediately
potassium EDTA anticoagulated blood
1 to 2 mg EDTA / 1ml blood
smears may be made up to two to three hours after collection
specimen must be free of clots
Blood smear – performance

make sure no clots are present in the specimen
if clots are present DO NOT use this specimen

place a small drop of blood on the surface of a clean glass slide near the end

if blood is taken from the finger, touch the drop of blood carefully to the slide and avoid touching the slide to the skin

if the blood is obtained by needle and syringe, depress plunger to allow a drop of blood to fall onto the end of the glass slide

hold the corners of the frosted end of the glass slide down on a flat surface with the thumb and index finger of one hand

with the other hand hold the second glass slide with the thumb and index (or great) finger at a 45° angle

place an edge of the spreader slide on the first slide to the left of the drop of blood and pull it to the edge of the drop

angle between the two slides will vary according to the size of the drop and the viscosity of the blood

The larger the drop and the lower the hematocrit of the blood are, the greater the angle must be to avoid
running off the slide when spreading.

Blood with a high hematocrit must be spread with a lower angle or the smear will be short and too thick to allow differentiation of cells

The approximate angle for normal blood is 30 to 40 degrees

Rest the spreader slide against the first one, and move it back carefully into the drop of blood

The blood will follow the edge of the slide evenly across the glass

If the smear is too thin, the angle of spreading should be increased
